Premises licence forms and guidance

Licensing Act application forms and guidance notes are available to download below.

Forms must be either by post or in person to the Licensing Authority, Caerphilly County Borough Council, Penallta House, Tredomen Park, Ystrad Mynach, Hengoed. CF82 7PG.

The Licensing Act 2003 also requires that certain applications be copied to a list of Responsible Authorities. These responsible authorities include the Police, planning authority, fire authority and trading standards.

Apply online: Please remember you can also apply online for premises licences via the GOV.UK website. Go to premises licence section and the links can be found under the heading 'How to apply'. If you apply online, the responsibility of serving copies on the relevant Responsible Authorities, lies with the Licensing Authority and not the applicant.

Variation application

Notice: The application to vary a premises licence to specify an individual as designated premises supervisor under the Licensing Act 2003 asks the licensee to give a copy of the form to the existing premises supervisor. The completed form contains personal information about the proposed new DPS and sharing this information would be in breach of the Data Protection Act 1998. It is sufficient for the licensee to inform the DPS that the application has been made, without the need to share the specific details of the application. A full copy of the application form must still be sent to police. The form will be amended via regulations as soon as possible to make this clear.
