Support and therapy at your home

Reablement is a short-term programme of intensive support and therapy in your own home. Support may involve input from Occupational Therapists, Physiotherapists, Nurses, Case Managers and trained Support Workers, to maximise your functions and enable you to continue living in your own home, rather than remaining in hospital or being admitted to long-term care.

If you have been assessed as requiring reablement support, the service you will get will depend on your needs with the focus on helping you to be as independent as possible. This could include: 

  • kitchen skills
  • mobility
  • safety at home
  • Practical advice on how to access the wider community including shopping, leisure and education facilities 

Our main aim is to work with you to help you become as independent as possible with every day activities. 

A member of the team will visit you to agree your goals and plan your day-to-day visits. The time of day and length of each visit will depend on your needs and will change, as you get closer to your goals and become more independent, the level of support we provide will reduce.

How long will the service last?

This is only a short-term service and will finish when you reach your goals. The time it takes will be different for each person but the longest we are able to work with you is a maximum of 6 weeks.

What happens when the service finishes?

We hope that you will not need any more support from us. If you do need more support, we will contact the correct team or agency to arrange any ongoing care and support you require.
